All Young Americans cast members are required to submit certificates of health due to our program’s intense physical and mental requirements.

We recommend scheduling an appointment with your doctor as soon as possible because it may take 2-3 weeks before you can get into their office. (TIP: We recommend getting your TB Test done at the same time to save yourself an appointment!)

After you have downloaded the forms below and gone to the doctor, upload the completed document to the Enrollment “Application” in Populi.
